The drug safety update contains omissions and contradiction in respect to the bibliography and the actual use of metamizole in healthcare practice. The official documents show a lack of updating, unapproved indications and doses higher than those recommended. The drug safety update has not stopped the presentation of cases of agranulocytosis due to metamizole.

The AEMPS drug safety update can be improved and it is necessary to update the official information documents on metamizole for health professionals and patients in order to decrease the risk of agranulocytosis.

It consists primarily of CD4(+) lymphocytes, a prominence of the T helper (Th)2 subtype but also Th1 cells, with Th17 cell-derived cytokines elevated in plasma. There are also neutrophils, eosinophils, basophils, and monocytes. Chemokines derived from mast cells and activated endothelial cells drive the process. Although the role of the cellular infiltrate has not previously been addressed, each constituent can contribute to the overall pathogenesis. It is of interest that CSU responds to corticosteroid, yet, short-term steroids do not affect autoimmunity or degranulation of mast cells, and act on margination of cells along the endothelium and chemotaxis to enter the surrounding dermis. In this review, we address each cell’s contribution to the overall inflammatory response, as it is currently understood, with a view toward development of therapeutic options that impede the function of critical cells and/or their secretory products.

This paper presents a new framework for automatic classification of sleep stages using a deep learning algorithm from single-channel EEG signals. Each segmented EEG signal appended with its label of stages is fed into a deep learning model to create an automatic sleep stage classification. This is one of the most important problems that is critical to the realization of monitoring patients with sleep disorder.

In the present study, a neural network architecture is introduced utilizing Convolutional Neural Networks (CNNs) to extract features, followed by Temporal Convolutional Neural Network to extract the temporal features from the extracted features vector of CNN. Finally, the performance of our model is improved by a Conditional Random Field layer. We also employed a new data augmentation technique to enhance the CNNs training which has auxiliary effects.

We evaluated our model by two different single-channel EEG signals (i.e., Fpz-Cz and Pz-Oz EEG channels) from two public sleep datasets, named Sleep-EDF-2013 and Sleep-EDF-2018. The evaluation results on both datasets showed that our model obtains the best total accuracy and kappa score (EDF-2013 85.39%- 0.80, EDF-2018 82.46%- 0.76) compared to the state-of-the-art methods.

This study will possibly allow us to have a wearable sleep monitoring system with a single-channel EEG. Also, unlike hand-crafted features methods, our model finds its own patterns through training epochs, and therefore, it may minimize engineering bias.

Previous studies investigated the feasibility of using house dust and wristbands to assess human OPE exposure. We hypothesized that these two approaches could differ in relative effectiveness in the characterization of children and adult exposure. In the participants recruited from Guangzhou, South China, urinary levels of major OPE metabolites, including diphenyl phosphate (DPHP) and bis(butoxyethyl) phosphate (BBOEP), were significantly higher in children than their mothers (median 6.6 versus 3.7 ng/mL and 0.11 versus 0.06 ng/mL, respectively). The associations of dust or wristband-associated OPEs with urinary metabolites exhibited chemical-specific patterns, which also differed between children and mothers. Significant and marginally significant associations were determined between dust concentrations of triphenyl phosphate (TPHP), tris(2-butoxyethyl) phosphate (TBOEP), trimethylphenyl phosphate (TMPP), or tris(1-chloro-2-propyl) phosphate (TCIPP) and their metabolites in children urine and between dust tris(1,3-dichloroisopropyl) phosphate (TDCIPP), TPHP or TMPP and urinary metabolites in mothers. By contrast, wristbands exhibited better efficiency of predicting internal exposure to TDCIPP. While both house dust and wristbands exhibited the potential as a convenient approach for assessing long-term OPE exposure, their feasibility requires better investigations via larger-scale studies and standardized sampling protocols.Carbamazepine (CBZ) decay by electro-Fenton (EF) oxidation using a novel FeS2/carbon felt (CF) cathode, instead of a soluble iron salt, was studied with the aim to accelerate the reaction between H2O2 and ferrous ions, which helps to produce more hydroxyl radicals (•OH) and eliminate iron sludge. First, fabricated FeS2 and its derived cathode were characterized by scanning electron microscopy, high-resolution transmission electron microscopy, and X-ray photoelectron spectroscopy. Anodes were then screened, with DSA (Ti/IrO2-RuO2) showing the best performance under EF oxidation regarding CBZ degradation and electrochemical characterization. Several operating parameters of this EF process, such as FeS2 loading, current density, gap between electrodes (GBE), initial [CBZ], and electrolyte type, were also investigated. Accordingly, a nonconsecutive empirical kinetic model was established to predict changes in CBZ concentration under the given operational parameters. The contribution of different oxidation types to the EF process was calculated using kinetic analysis and quenching experiments to verify the role of the FeS2-modified cathode. The reaction contributions of anodic oxidation (AO), H2O2 electrolysis (EP), and EF oxidation to CBZ removal were 12.81%, 7.41%, and 79.77%, respectively. The •OH exposure of EP and EF oxidation was calculated, confirming that •OH exposure was approximately 22.45-fold higher using FeS2-modified CF. Finally, the 19 intermediates formed by CBZ degradation were identified by ultra-performance liquid chromatography/quadrupole time-of-flight mass spectrometry. Accordingly, four CBZ degradation pathways were proposed. ECOSAR software was used to assess the ecotoxicity of intermediates toward fish, daphnia, and green algae, showing that this novel EF oxidation process showed good toxicity reduction performance. A prolonged EF retention time was proposed to be necessary to obtain clean and safe water, even if the targeted compound was removed at an earlier time.Trait-based approaches are promising to make generalizations about the sensitivity of species and populations to pesticides. Two traits that may shape the sensitivity to pesticides are the surface area (related to pesticide uptake) and the metabolic rate (related to pesticide elimination). We compared the sensitivity of damselfly larvae to the pesticide chlorpyrifos and how this was modified by loss of external gills (autotomy, reducing the surface area) in both fast pace-of-life (high metabolic rate) and slow pace-of-life (low metabolic rate) populations of Ischnura elegans. The slow-paced populations were more sensitive to the pesticide than the fast-paced populations in terms of survival, growth and energy metabolism. This suggests the higher metabolic rate of fast-paced populations enabled a faster pesticide elimination. Pesticide exposure also reduced heat tolerance, especially in slow-paced larvae under hypoxia. Gill loss had opposite effects on pesticide sensitivity in slow- and fast-paced populations. In slow-paced larvae, gill loss lowered the sensitivity to the pesticide, while in fast-paced larvae, gill loss increased the sensitivity. This difference likely reflects the balance between the roles of the gills in pesticide uptake (more detrimental in slow-paced populations) and oxygen uptake (more important in fast-paced populations). Our results highlight the need to consider trait interactions when applying trait-based approaches to predict the sensitivity to pesticides.Summer intensive air measurements of alkylated polycyclic aromatic compounds (Alk-PACs), nitrated polycyclic aromatic hydrocarbons (NPAHs), and oxygenated polycyclic aromatic hydrocarbons (OPAHs) was conducted during the summer of 2013 at an air monitoring site near the community of Fort McKay in the Athabasca oil sands region (AOSR). This study uses the ambient air measurements in conjunction with supplementary meteorological and air quality data from coordinated ground- and aircraft-based sampling over the same period to characterize diurnal variations and changes in the organic air pollutant profiles associated with the plume episodes. Principal component analysis showed a distinct PAC profile during plume episodes, driven mainly by higher fluorenone (FLO) and 9,10-anthraquinone (ANQ) concentrations. During the plume episodes (August 23-24), means of NPAHs and OPAHs concentrations were 120 and 2020 pg/m3, respectively, which were 2.7 and 2.5 times higher than those measured on the other days, while Alk-PACthe region.Cadmium (Cd), as an environmental pollutant, can lead to nephrotoxicity. However, its nephrotoxicological mechanisms have not been fully elucidated. In this study, Cd (1.5 mg/kg body weight, gavaged for 4 weeks) was found to induce the renal damage in mice, based on indicators including Cd concentration, kidney index, serum creatinine and blood urea nitrogen levels, pro-inflammatory cytokines and their mRNA expressions, levels of Bcl-2, Bax and caspase9, and histopathological changes of the kidneys. Furthermore, Cd-caused detrimental changes through inducing inflammation and apoptosis via the miR-34a/Sirt1/p53 axis. This is the first report on the role of miR-34a/Sirt1/p53 axis in regulating Cd-caused apoptosis and nephrotoxicity in mice. The findings obtained in this study provide new insights into miRNA-based regulation of heavy metal induced-nephrotoxicity.This research explored the occurrence, epigenetic toxic profiling and main toxic pollutants of POPs in surface water of Dongjiang River, southern China. The concentrations of selected POPs including polycyclic aromatic hydrocarbons (PAHs), endocrine disrupting chemicals (EDCs), phthalate esters (PAEs) and polybrominated diphenyl ethers (PBDEs) of surface water from 18 sites were investigated. ∑16PAHs and ∑4EDCs were at a moderate level, while ∑6PAEs and ∑6PBDEs had low pollution levels. PAHs, EDCs and PAEs showed higher concentrations in dry season than those in wet season, and the loading of selected POPs in tributaries was higher than those in mainstream due to intensive manufactures and lower runoff volume. Moreover, activities of DNA methyltransferase (DNMT)1, histone deacetylase (HDAC2, HDAC8) were confirmed to be sensitive indicators for epigenetic toxicity. The DNMT1-mediated epigenetic equivalency toxicity of organic extracts in Dongjiang River were more serious than those of HDAC2 and HDAC8. Correlation analysis shown binding affinity between POPs and DNMT1, HDAC2 and HDAC8 could be regarded as toxic equivalency factors.
